**Care Coordinator**

Contract: 16-20 hours part time

**Salary**: £21,000 - £22,000 per year (pro rata) depending on relevant qualifications (minimum NVQ Level 2 in Health & Social Care - working towards Level 3) and minimum 2 years care experience.

We are seeking a **Care Coordinator** to join our expanding team.

You will have access to a pension scheme and mileage/expense payments.

The company offers a 4-day training induction and a detailed training programme.

**As the Care Coordinator your duties will include**:

- Client reviews - Supporting all clients requirements;
- Working closely with the Care Manager and other Coordinators to improve the services the company provide;
- Shadowing and introducing staff in Client calls;
- Staff supervisions and competence assessments;
- Delivering care as required by the needs of the business;
- Completing records and inputting them onto the system;
- Organising team meetings;
- Completing weekly reports.

You will have on-call responsibilities where you will liaise with staff, clients, relatives and relevant authorities and complete associated tasks as required.

**To be a successful Care Coordinator you will have the following skills and experience**:

- Full UK driving license with access to a car is essential;
- NVQ Level 2 in Health and Social Care and looking to work towards an NVQ Level 3;
- Excellent Communication & Computer skills;
- Positive role model;
- A good Team builder;
- Ability to cope with pressure;
- Able to manage own workload;
- Be flexible, adaptable and able to work effectively in a variety of settings.

You will be required to work 16-20 hours (part time) per week, generally within office hours; however, to meet the needs of the business, some early starts, late finishes and weekend working will be required. (Generally between 7am - 11 pm Monday to Sunday). You will also be part of the on-call rota.

If you are looking for full time hours, this can be supplemented by delivering care outside of your Care Coordination hours.
