**Health Care Assistant - Community**

Sincerity Health Care provides care and support services to the elderly and people with autism and learning disabilities. We work with individuals, families and service commissioners to support people to live within the community, either in their homes or in supported living service.

Our aim is to provide support services to vulnerable adults and individuals with autism, learning disabilities and associated needs in an environment where rights, independence and choice are our focus - we provide person-centred care and support, which is flexible and responsive.

- Reporting to the Team Leader and Care Coordinator
- This role requires flexible hours over the whole week (weekdays, weekends and may include nights)
- You will provide direct and person-centred care support to the elderly and adults with Autism and learning disabilities.
- This position will also include personal care duties when needed

As part of your role, as a Health Care Assistant you will be expected to carry out duties such as:

- Meal planning, activities, community support, along with any further daily living tasks as required. You will be caring, compassionate, have good work ethics and be able to work on your own initiative, whilst following the support and care plans provided for the people we support.

**Job Requirements**:

- Previous experience, ideally within a similar role - working with elderly and adults with learning disabilities.
- Be able to complete mandatory training and be prepared to utilise this, when required.
- Fully flexible, across 7 days as all shifts are worked on a rota basis. Be willing and able to participate in daily activities.
- Be able to follow a detailed care plan along with completing daily logs.
