Care Coordinator/team Leader

DarSarno Care Services specialises in providing home-care and end-of-life care services to adults in the community. We believe that we can make a difference in our client’s lives through the services we provide. We are looking for a dedicated, committed, and trustworthy individual to join our team of caring staff as a team leader or care coordinator.
The post will involve working with staff, other professionals, Service Users’ in their homes providing them with support for their daily needs (domiciliary/personal care).
**Essential Skills we require**:
- Previous staff supervising experience - this could be in a Senior/Team Leader/Field Care Supervisor or Care Coordinator role.
- Experience and knowledge in Health and Social Care.
- QCF in Health and Social Care desirable or working towards.
- We are looking for an individual with the ability to work in a community environment providing mainly personal care in Daventry and surrounding areas and leading other staff members.
- Willing to work as part of a team, team leader and as a lone worker.
- Computer literacy skills.
- Good communication skills.
- Experience working with domiciliary or residential care.
- A valid UK driving license and access to a vehicle is a must due to the nature of the position.
- Excellent Customer Care Skills.
**General Responsibility**:
The following job description outlines the primary responsibilities of the Care Co-ordinator. Please note that this is not an exhaustive list, and the Company may modify this description as needed to meet the requirements of the business. Additionally, you may be required to perform other duties from time to time as directed by the Company.
As the Care/Staff Coordinator, you will,
- Accept responsibility for the professional supervision, management, organisation, and coordination of staff, including the management of the out-of-hours service in the absence of the Care Manager.
- Need to cover any short-term unassigned shifts also you will be required to have 2 to 3 days in the field and the other days in the office, making sure all clients are receiving excellent care daily.
- To reconcile the Electronic Call Monitoring System (where applicable), highlighting and investigating any issues that have arisen and reporting such issues to the Operations Manager.
- Support with the recruitment processes.
- To assist with the implementation of care plans, liaising with the clients and their families with regards to the details and the start date of their care package(s), and maintaining regular contact with clients and their families regarding their day to day care.
- To ensure that all services offered are appropriate, relevant, and sensitive to the needs of service users and to initiate change in line with their needs, in consultation with the Operations Manager.
- To take overall responsibility for the completion of rotas on a weekly basis to ensure that service user calls are appropriately planned and covered by Care workers.
