Children & Transition Team Leader

Work in partnership with key partners and stakeholders in the delivery of the service, particular the local Council Adult Social Care Departments and local Provider of NHS Services to support the delivery of the strategies, projects and delivery plans that form part of the ICB Operating Framework. Develop and maintain strong working relationships with partners which is cognisant of statutory responsibilities, budgetary restrictions and scope of practice. Using various communication methods, provide timely advice on highly complex situations, provide information or support to internal and external stakeholders around related function, adhering to policies, legislation, governance requirements and best practice guidance. Manage a clear dialogue regarding responsibility, accountability and meeting the needs of the population via the relevant eligibility, commissioning and funding arrangements in place.
Attend necessary Panels and provide a senior clinical support function to decision making. Ensure that team undertake robust and evidence based Section 117 Aftercare assessments for patients across all aspects of care. This necessitates a high level of assessment skills requiring tact, diplomacy and the ability to liaise effectively with colleagues, health care professionals, service users' carers and family members. Ensure personal health budgets are actively promoted with service users and their representatives and work closely with the wider team to encourage a seamless delivery.
Deal with incoming appeals, ensuring the appropriate actions are undertaken and the ICB procedures and policies are adhered to. Represent the ICB at meetings or proceedings in respect of Safeguarding Vulnerable Adult cases as required by line manager. Provide clinical supervision to the teams Nurses in respect of assessments and complex cases providing opportunities for reflection and identifying where further training is required. Audit and verify s117 Aftercare Matrix to ensure a consistent approach to decision making and to provide assurance where decisions have been made away from the MDT panel.
Act as an expert resource & provide professional advice on complex matters to Acute trusts, nursing homes, nursing staff, GP practices, social services and the ICB in respect of NHS Funded Healthcare. Ensure that Health systems and processes are developed and in place to support effective transition planning alongside social care colleagues. To ensure that all data held in regard to current caseload is effectively and correctively maintained. Produce timely function specific reports, papers and databases, using prolonged concentration and appropriate software packages when required.
Ensuring that the Team works within the national and local legislation, policy and procedure. Ensure that any other relevant polices in line with general duties are implemented and adhered to e.g. Safeguarding Vulnerable Adults, reporting as per Local Authority requirements.
