As a team we are blending medical innovation with bespoke care, bridging the gap between hospital and home. A virtual ward is a safe and efficient alternative to NHS bedded care that is enabled by technology and home visits. The innovative approach enables us to deliver safe, high quality and personalised care to people at home where they would rather be.

We know that people recover faster and better in their own environments and our dedicated teams of trained staff endeavour to support this. A hybrid model of virtual, telephone and face to face consultations can be delivered as required, allowing patients to recover in their own home with continued clinical support.

**What will you be doing?**

The Nurse Associates provide holistic, person-centred care and support for service users of all ages in a variety of settings. You will be bridging the gap between health care support and registered nurses, whilst working safely within the scope of practice and competence at all times.

Key responsibilities include:

- Undertaking holistic assessments
- Implementing care programmes
- Assessing and monitoring service users health needs

**What do you need?**
- Higher Apprenticeship in Health Level 5 or Foundation Degree in Health and Social Care
- Current NMC registration
- Full driving licence and access to car in work time
- Experience of working in a health care or social care setting providing direct patient care, demonstrating a clear understanding of how health can impact on all aspects of everyday life
- Ability to manipulate fine tools, e.g. for Venepuncture and other clinical tasks
- A clear understanding of NMC standards of proficiencies and professional standards of practice including Revalidation
- Ability to work unsupervised
