**Contracted Hours**:37 (part-time hours will also considered)

**Job Type**:Permanent

**Salary**:£21,000 - £23,000 per annum

**Location**:Dementia Centre and flexible working

**Probationary Period**:3 Months

**Job Summary**

The post holder will report to the Dementia Support Manager and form part of the

Dementia Support team for Approach.

The post holder will act as a central point of contact between people affected by dementia and Approach’s services. This will include ensuring that people have access to the right service offered by Approach and its partners at the right time and upholding a person
- centred care approach.

Working closely with the Dementia Support Manager, the post holder will engage with people affected by dementia in the community, in particular during Carer Support and

Advice sessions and across the pathway of dementia services offered by Approach and its strategic partner.

**Duties and Responsibilities of Post Holder**:
**Service Delivery**

1. Working closely with the Dementia Support Manager, be responsible for the efficient and effective referral of appropriate dementia support and group dementia support.

2. Develop strong links with external partners and organisations (such as GPs, social prescribers and Admiral Nurses) to enable the effective referral of people with dementia to services to improve their wellbeing, enabling a network of support from health and social care professionals.

3. Initiate, develop and maintain strong and effective working relationships with other dementia professionals working in the local dementia landscape.

4. Build awareness of activities and support provided by Approach and its strategic partners.

5. Manage a caseload of individuals to provide effective support and guidance during their dementia journey from early diagnosis through to later dementia stages.

6. Proactively work to minimise obstacles for local people to access dementia support.

7. Provide expertise and guidance for people affected by dementia on a range of issues which impact their care such as lasting power of attorney and guidance in completion of relevant statutory funding provision such as attendance allowance.

8. Working closely with strategic partners, ensure that the right support is provided to people at the correct stage in their dementia journey through referrals or signposting.

9. Act as an ambassador for Approach and its strategic partners to ensure continued awareness of Approach and its services.

10. Promote positive attitudes by providing information and support to raise awareness of the practical, emotional and psychological impacts of living with dementia.

11. Ensure individual person-centred care plans, risk assessments and record keeping are carried out effectively, are regularly maintained and updated and are compliant with recognised standards.

12. Working closely with the administration team, ensure timely compilation and submission of quality monitoring statistics and reports as required.

13. Working closely with the Dementia Support Manager, contribute to the development of strategic plans for operational activity as required. Implement operational plans as required and identify gaps within current service provision.

14. As a team, implement systems and processes for audit and recording to ensure quality service delivery.

15. Monitor and record service performance and implement improvements where applicable whilst informing the Dementia Support Manager in a timely manner of issues arising and actions taken/required to enable effective evaluation.

16. Report to the Head of Charity and Board of Trustees in a timely manner any concerns and or identified weaknesses in service delivery together with actions taken/required.

17. Assist in the delivery of Carer Support and Advice sessions and Dementia Group
Support as required.

18. Attend and Chair staff meetings as required.

19. Attend meetings on behalf of the organisation. Develop positive and efficient lines of communication with the general public, statutory and voluntary organisations to raise awareness of and promote the use of Approach services.

20. Provide support to service users and monitor the co-ordination of funding arrangements for individuals to access services.
