Following a redesign of services, we are in an exciting position to be able to recruit an Associate Practitioner as part of our Dementia Services in the South East Essex area.

We are seeking an Associate Practitioner to work in partnership closely with a Dementia/Mental Health Nurse Specialist across CPR and Southend Localities, will provide assistant to community Dementia and Older Adult Mental Health Services, post-diagnostic support, dementia reviews, triaging referrals and passing to the appropriate teams. They will also offer advice and support to other professionals and advice the MDT on referrals.

The post holder will also be working closely with Community Dementia Nurse Specialists and other Senior MDT members to offer assessment, treatment and provide care and support to people with a diagnosis or suspected diagnosis of Dementia and their Carers within community settings and/or the patient’s own home.

This is an important role working as part of a team with a friendly atmosphere and strong work ethic.

As an Associate Practitioner within the Dementia Services, you will be required to work independently with service users under an agreed plan of care, formed with Senior team members. You will support initial care planning, when necessary and update plans as needs and circumstances change, while seeking suitable supervision.

You will also need to have the ability to work under pressure and although a key part of the team, on your own initiative.

**The post holder will**:
Work in partnership with the Community Dementia Nurse Specialist and other Senior MDT members, to offer assessment and treatment to service users under the care of the Dementia Services

Work independently with Service Users under an agreed plan of care, formed with Senior Team members

Support initial Care Planning, when necessary and update plans as needs and circumstances change, while seeking suitable supervisio

Assist and contribute to the Assessment process, Initial Care Planning, Risk Assessment and Review of patient care

To refer Service Users to other services, both internal and external to EPUT

Use own initiative to plan and implement Social/Therapeutic activities for Service Users, sign posting to local community resources and supporting client engagement

Undertake Physiological measurements and observations of Service Users’ Physical and Mental Health conditions in order to recognise significant changes and be able to report and act on, as appropriate

To collect specimens, including the taking of blood and ensuring that all routine and prescribed investigations are completed

To complete ECGS, take TPR and BP when required

Provide post diagnostic support and education to service users and their families referring to appropriate services (such as Dementia Navigation)

Contribute to the discharge and care pathway planning
