We have collaborated with a local GP surgery to be part of a project supporting patients living with Dementia.

We’re looking for a highly motivated, dedicated, and experienced person to lead on 21 week project

You will be required to contact patients and their carers, who are living with dementia as part of their routine reviews. It will involve collecting information, listening, and supporting with concerns and signposting to other services, including the Dougie Mac Admiral Nurse service and Approach Dementia Support services. This role can be worked alongside your current role. Full Job Description Available

**Salary**: £12.97 per hour

**Hours**:22.5 hours a week ¦ Day shifts
