Main Responsibilities

The successful candidate will be responsible for delivering high-quality mental health services to patients across all ages, working alongside the existing Primary Care Network (PCN) based mental health roles. This will involve providing timely access to an integrated, user-centred service that enables people with mental health needs and/or social care problems to improve their well-being and functionality.

Key Duties
  • Undertake assessments of individual clients' difficulties, establishing risk and determining the appropriate course of action.
  • Proactively identify and manage treatment plans for patients with dementia and serious mental illness.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.
  • Take prompt action on changes in condition, liaising with other caregivers, members of the clinical team and/or other agencies when necessary.
Requirements

The post holder will be an experienced mental health practitioner with a proven track record of working in a Primary Care and/or Community Mental Health Team setting. They will demonstrate outstanding autonomous and critical thinking, and be a caring, compassionate and committed experienced mental health professional.

Experience
  • Significant post qualification experience in mental health practice.
  • Ability to work across boundaries of care.
