**SALARY **£22,800 - £25,000

**Purpose of Job**

To support families and carers affected by substance misuse. To develop and deliver interventions and care plans according to the systems, measures and processes of the organisation.

**Main Responsibilities**
- To manage an allocated case load, working with families and carers affected by substance misuse located in the Blyth area.
- To develop, co-ordinate and deliver appropriate family interventions to families and carers affected by substance misuse
- To assess the needs and risks within families where substance misuse has been identified as an issue and identify the most suitable and appropriate care package whilst ensuring outcome measures are met
- To work closely with the treatment provider to promote ESCAPE work and ensure we reach families that require support.
- To work with ICCQ partners to develop initiatives to support hard to reach families in Blyth.
- To develop and maintain relationships with families and carers on a one to one basis or as a group unit
- To assess the needs and risks within families on an individual needs basis
- To promote a greater understanding of family and carer dynamics and how this impacts clients
- To work closely with other case workers delivering packages of care to families who need additional support
- To facilitate group work and training with families affected by substance misuse
- To proactively communicate to all relevant partners and bodies to ensure effective dissemination of information within the parameters of confidentiality
- To contribute to the information and monitoring systems which will assist in the further development of appropriate services
- To complete regular and reliable case notes, assessment reports and care plans, ensuring professional, legal and organisational processes and systems are adhered to
- To carry out all duties with due regard to quality, health, safety and equal opportunities as defined in the organisation’s policies, practices and procedures Staff Handbook
- To meet the agreed performance criteria as outlined and agreed in minimum yearly appraisals
- To embrace and uphold the values of the organisation, consistently demonstrating the behaviours which underpin these values
- To maintain the highest standard of professional conduct at all times
- To undertake any other tasks and responsibilities appropriate to this post as the organisation requires within the changing environment of this sector

**Reporting and Authority**
- This role reports to the Deputy Chief Executive & Service Manager.
- There will be a need to accept responsibility for guiding, leading and supervising volunteers as designated.
- The role requires decisions to be made as to the most appropriate service solution to be offered to clients as within the standard operating procedures.
- This role requires the role holder to self-manage their time and workload, in line with their work commitments and direction from the Deputy Chief Executive & Service Manager.
