**MAIN PURPOSES OF JOB POSITION**
- To work as part of a team with colleagues in meeting the personal care needs of the service users within the home, respecting their dignity, and promoting independence at the same time.
- To maintain professionalism with health and social care professionals.
- To assist in the care of the service user’s physical environment and in the general day to day activities of the home.
- To take delegated responsibility of the service users under your care to the appropriate level determined.
- To provide 24-hour supervision of service user physical and mental health needs when on duty.
- Record all care related interactions on nourish.
- To make sure a set of building keys, nourish device and landline is carried at all times during shift.
- Maintain cleanliness, hygiene and tidy environment free of clutter, ensuring equipment, COSHH items, personal phones, bags, jackets and belongings are appropriately stored away and black bags and rubbish is disposed of immediately.
- Ensuring appropriate PPE is worn and always stocked throughout the building.
- Ensuring COSHH items used appropriately and correctly stored away after use.
- Ensure uniforms washed in between each shift and always maintain a professional appearance on duty, badges must be worn. (No hoodies, jewellery, nail polish or extensions, hair tied back, black trousers only - leggings and black shoes/trainers not white).
- To carry out regular 15 - 30 minutes checks on service user’s who display relapse signs and record on nourish.
- Contact relevant health and social care professionals when service users display relapse signs immediately.
- To respond to nurse call immediately, assess the situation and assist the service user seeking help accordingly.
- Staff must sign in and out of shift.
- Leaving personal circumstance outside the work parameters.

**MAIN RESPONSIBILITIES & KEY TASKS**
- To arrive 10 minutes prior to shift for handover to ensure you are up to date with relevant changes to service user’s needs.
- Delegated responsibilities on shift to be shared by management or Team Coordinators.
- To assist service users who need help with any aspects of personal care as appropriate. To ensure personal care needs are met to a high standard and monitored throughout shifts for any additional needs such as sores, clothing changes, continence, and cleanliness of environment.
- To assist service users who may need help with mobility and other physical problems using appropriate aids and personal equipment. Ensuring equipment is correctly serviced and cleaned after every use.
- Supporting and prompting service user’s to attend regular care reviews, appointments and consistently attend chiropody, dentists and opticians.
- Family to be informed of changes and updates about service users where relevant and consented.
- Ensuring appropriate daily observations and measurements are completed and recorded e.g. Leg Measurements, BP, Weights.
- To support service users to make their beds, tidy bedrooms and including domestic duties within their room e.g. bathrooms, bedrooms etc. Prompting service users to support with such activities where possible to promote their independence.
- To promote the independence of service users to their maximum capabilities, through freedom of choice and maximising their independence in all activities of daily living.
- Monitor and supervise service user’s smoking habits, supporting where necessary e.g. lighting cigarettes and supervising in instances of risk of harm to self.
- To promote health, personal, cultural, religious, and social needs of service users e.g. attending religious temples, celebrating religious and cultural festivities and prompting individual’s religious dietary needs.
- To mend / sew clothing of individual service users as necessary.
- To assist and observe service users with their personal laundering i.e. washing, drying and ironing and to assist them to put clothing away neatly in their wardrobes.
- To assist and observe in serving meals and assist service users to prepare snacks and drinks for themselves.
- To assist and observe those service users that require support / assistance with eating and drinking.
- To assist and prompt service users with clearing the dining room and ensure that the dining room floor is swept, vacuumed, mopped and tables cleaned - covers replaced.
- To answer the telephone in a professional manner and take / record messages correctly.
- To be able to take any enquiries and deal with them in an efficient and professional manner. Ensuring these are appropriately recorded and actioned accordingly.
