We are looking for a Band 6 nurse, social worker or occupational to join our team in Brent as a community clinician/mental health practitioner.

As part of our award-winning Health Inequalities team; you will connect with the local community to plan, develop and deliver interventions to the most vulnerable and marginalised groups in the borough.

The post holder will be from Brent or have very close links with Brent. You will have good connections within local communities and have a real desire for helping people.

You will work closely with GPs to identify access issues and barriers affecting the GP population leading to poor or low uptake of services designed to support their health and wellbeing.

You will use a population health-based approach with a focus on the wider determinants of health to support GPs tailor their intervention to address current and future needs of their population.

Your social work training will underpin and inform the assessment, the care and support planning or interventions that you deliver to groups and individuals.

You will work closely with GPs to identify access issues and barriers affecting the GP population leading to poor or low uptake of services designed to support their health and wellbeing.

You will use a population health-based approach with a focus on the wider determinants of health to support GPs tailor their intervention to address current and future needs of their population.

Your professional training will underpin and inform the assessment, the care and support planning or interventions that you deliver to groups and individuals.

A substantial part of your working time will be based within the local communities working closely in identifying health and wellbeing needs and supporting the development of services in response to those needs. Part of your role will be seeking the community voice; recognising and addressing concerns about the physical health and mental needs of service users with long term conditions (e.g. diabetes, asthma heart disease, anxiety and depression, loss and bereavement and other health and wellbeing needs etc.) Advising the community on prevention of illness and uptake of preventative measures with focus on choice, self-management and social inclusion.

You will work closely with voluntary sector organisations and the local public health team to plan and deliver mental health and wellbeing support to those who have been affected by the coronavirus pandemic and experience health inequalities.

Brent health inequalities team is an established ward-winning team with a friendly, supportive culture. It is part of the broader Brent Health Matters Programme which was set up to support the well-being needs of Brent residents and tackling and reducing inequalities. There is a visible and supportive management team and a strong emphasis on training and professional development opportunities.

- To provide regular ongoing contact with GP Practices as part of the post holder’s link worker role, this

includes developing an improved understanding of the role and function of the CMHT.
Key Responsibilities
- Work collaboratively with other members of the multi-disciplinary team in the delivery of MH and Wellbeing

care pathway.
