**JOB DESCRIPTION**
**POST TITLE**: Dementia Service Facilitator
**CONTRACT DURATION**
**SALARY**:£12 per hour
**HOURS**: 6 hours per week in total: 1pm to 4pm, 2 days per week (Tuesdays and Wednesdays)
**LOCATION: Wimborne**
**RESPONSIBLE TO**: Dementia Service Manager
Head of Operations ,Chief Executive and ultimately Trustees of Age UK Bournemouth, Poole & East Dorset
**JOB PURPOSE**: To facilitate/deliver a programme of engaging, fun and stimulating activities and social workshops to improve memory, cognitive function and quality of life for people living with mild to moderate dementia, their carers and families. You will have the support of an assistant and or volunteer.
**MAIN TASKS**
1. To plan and facilitate a pre-designed group activity for a class of 6-10 members working to
a 14-week program for those with mild-moderate dementia, or those with cognitive
issues.
2. Ensure service delivery is run smoothly, effectively and in line with relevant quality standards.
3. Room set up and cleaning down of equipment after each session. Ensure venues are tidy and secure at the end of each session.
4. Contact clients who don't attend sessions.
5. To complete evaluation and monitoring forms at the first and last session.
6. To assist with refreshments.
7. Report any safeguarding concerns and ensure the health and safety of those attending.
8. Responsible for any staff volunteers.
9. To undertake all necessary training.
10. To adhere to all Age UK Bournemouth, Poole and East Dorset policies and procedures.
11. To undertake any other reasonable duties as requested by your Line Manager, Senior
Managers, Chief Executive or Trustees.
