A fantastic opportunity has arisen to work in our Hammersmith & Fulham (H&F) Community Rehabilitation Service (CRS) as a B6 Substance Use Practitioner. This role is a part-time position, working 2 and half days per week. This post also attracts 20% inner London weighting.

The H&F Adult Community Rehabilitation Team provides support for people with complex care needs related to psychosis and (where relevant) co-existing substance use and physical health problems. The service provides a less restrictive alternative to inpatient rehabilitation by providing intensive support for service users so that rehabilitation occurs in the community. With a multidisciplinary and whole-team approach, the emphasis is on delivering individualised and recovery-focussed care.

H&F CRS is based close to public transport with a short walk to the office. The area is vibrant with access to various retailers, food outlets and the all-important coffee shops

The post holder will work as a member of the MDT as a dual diagnosis specialist. They will offer assessment, consultation and support for patients under the care of the team.

The post holder will work in close collaboration with patients, families and other professionals and services involved in the person’s care, including acute trust colleagues, mental health trust teams, primary care, social services, housing and homelessness services, substance use services, schools and colleges, ambulance services, the police and other criminal justice agencies.

To support the development of a new service aimed at meeting the needs of patients under the care of our Community Rehabilitation team who have complex conditions relating to serious mental health difficulties and co-existing substance use issues.

To support the development of evidence based practice in all aspects of clinical care, ensuring an ongoing awareness of significant health research relevant to the care and treatment of patients with substance use and dual diagnosis.

To engage with individuals referred to the service utilising the principles of motivational interviewing to establish a therapeutic relationship.

To deliver training to staff within the Trust and CCG in relation to substance use and dual diagnosis.

Community Rehabilitation Services within West London NHS Trust was established in early 2021. Three teams were formed across 3 different boroughs: Ealing, Hounslow and Hammersmith and Fulham (H&F). The service is suitable for those who are aged 18+ years (no upper limit).

The service is multidisciplinary and each team consists of a Consultant Psychiatrist, Team Lead, care co-ordinators, OT’s, Psychologists, support workers and an administrator.

The service was designed to provide a less restrictive alternative to inpatient rehabilitation by providing intensive support for service users with complex psychosis so that rehabilitation can occur in the community.

A key function of the rehabilitation teams is to support individuals who have been placed outside of the borough to move back to their local area.

For service users in or in need of 24 hour supported accommodation, or those who have frequent or lengthy inpatient stays, the service provides intensive MDT support and close partnership working with housing and community providers to stabilise mental health, maximise independence and functioning and support people to flourish in least restrictive community settings in accordance with their goals and preferences.

The post holder will use an integrated motivational interviewing approach that is person-centred and holistic which considers the needs of others in the service User social system. They will be required to conduct a comprehensive biopsychosocial assessment which considers the relationship between the person’s mental and physical health, their social situation and their harmful use of drugs and alcohol in formulating person-centred and needs-based care plans.
