**Bramble Lodge** is a well renowned Residential and Dementia Care Home in West Hallam. Many of our staff are long servers, some having been with us for over 20 years.

**Role: Care Coordinator - 40 Hours per week spread across 5 days**
**Reports to: Deputy Manager**
**Salary: £24,000 - £25,000 per year**
The purpose of this role is to ensure the delivery of highest quality care to all the residents complying with Health & Social Care Act 2008 (Regulated Activities) Regulations 2014. Care Coordinator will implement policies and procedures and embed consistency in good care practices and ensure the effective operation of delivery of person-centred care within the Home.
**Care & Quality Focus**
- Develop and maintain a person-centred approach care delivery and implement robust care plans including appropriate risk assessments.
- Monitor the day-to-day functioning of shifts and ensure compliance with comprehensive handovers.
- Ensure the continuity of care in line with the CQC’s 5 key lines of enquiry.
- Carry out regular audits and ensure all actions are fully identified & resolved in the required timeframe.
- Ensure a full and comprehensive clinical compliance through day-to-day monitoring of the service, meetings and feedback systems.
- Able to teach, train and manage the care team to ensure safe and effective care.
- Able to maintain high staff morale and communicates effectively to support and retain staff.
**Skills, Experience, Qualification & Knowledge**:
- Essential - Previous experience in a leadership role within a care home setting.
- Essential - Level 3 qualified and/or equivalent qualifications in Health & Social Care.
- Successful track record of improving the quality of service.
- An in-depth understanding of Health & Social Care Act 2008 (Regulated Activities) Regulations 2014.
- A thorough understanding of CQC key lines of enquiry and inspection methodology.
- Proficient is use of Microsoft Word, Microsoft Excel and ability to learn new IT systems.
- Effective communication, time management and leadership abilities.
