We are looking for **dedicated**, **enthusiastic** and **creative** individuals with a can-do approach to join our expanding teams of support workers in Leicester.

Pathfinders Community Support is a community based service supporting individuals in their homes who have learning disabilities, physical disabilities, mental health needs, profound and complex needs / behaviours that may challenge.

Services include supported living, day services, domiciliary care services and transforming care transitional support. Pathfinders Community Support Ltd is registered with the Care Quality Commission and complies with the Domiciliary Care Regulations.

**Purpose of the role**:
As a Team Leader you will provide and monitor safe, effective, well-led and quality service to both service users and staff teams.

Provide leadership, guidance and supervision to the team of support workers providing culture of team work.
- Reflect the provision of 24/7 care as necessary
- This will involve office hours / clinical hours delivering some direct support to individuals
- This includes working evening, weekends, bank holidays and other unsociable hours as required to meet the needs of the people we support and support our staff team.
- 24/7 on-call service on a rota basis
- Enable People we support to make choices to encourage their independence & social inclusion.
- Inform the Manager of any staff related issues, which may impact upon the quality of the service provided.
- Report any incidents, accidents, illness and risks which may affect safety in accordance with procedures.
- Ensure safeguarding responsibilities are met and to protect the adults’ rights to live safely and free from abuse or neglect.
- Leading and mentoring support staff

**KEY RESPONSABILITIES**

1. To support the Registered Manager to oversee supported living service, flexible short breaks and community opportunities, ensuring that systems and standards are maintained to a high quality.

2. To ensure compliance with all Acts, Legislations and external regulatory standards in line with and appropriate to the running of Supported Living / Domiciliary Care agency registered with CQC to include Care Act 2010.

3. Conduct team meetings with the Supported Living Manager.

4. Attend keyworker meetings and represent the service user.

5. Manage sickness absence and annual leave requests.

6. Produce staff rotas, ensuring staffing levels are maintained, source staff as required and carry out care duties as part of the rota.

7. Ensure daily plans, documents and weekly audits are completed.

8. Order and audit medication.

9. Carry out observational supervision.

10. Note taking for investigations.

**The person**

1. Proven background in a social care setting as a senior or team leader, or a skilled support worker who has supervisory experience and is looking to take on a leadership role

2. Preferably NVQ Level 3 Health and Social Care or working towards this

3. Excellent level of English

4. Clear and concise verbal and written communication skills are essential along with strong organisational skills.
