Principal Job Purpose: The job-holders principal purpose will be to support the strategic and operational delivery of the development of the Palliative and End of Life Transitional Care Pathway undertaking the planning, monitoring, and reporting of the project, working with a range of stakeholders across South Yorkshire. The project will be a feasibility study to determine the potential for a workable patient pathway to be found around the issue of transition to palliative and end-of-life services for children and young people in SYB within primary and secondary care including the hospice sector. Key Responsibilities: An overview of the job-holders key responsibilities include: - To manage, lead and take responsibility for the delivery of the project that encompasses the Palliative and End of Life Transitional Care Pathway from project scope to project close including where possible the follow on workplan.

To scope, understand and meet the needs of the project across the organisations and with stakeholders ensuring these are mapped and managed throughout the project lifecycle. To manage, co-ordinate and direct resources assigned to projects on a day-to-day basis to deliver the project's objectives ensuring optimal value is achieved To produce all project documentation including detailed project plans/work packages, resource and risk plans, exception reports and end stage reports to enable effective project governance, sponsor and stakeholder oversight. To develop and refresh communications plans (at key project stages) to enable timely information out and an engaged approach to delivery To report and present on project delivery to relevant Task and Finish Group and Children & Young Peoples Programme Board and Stakeholder groups To advise and influence internal and external service users and commissioners at all levels, regarding the project delivery to enable proactive and planned engagement, stakeholder and sponsor support, ownership, and risk mitigation. To develop effective professional partnerships with stakeholders, service providers and commissioners and liaise proactively to ensure sustained project delivery.

Be aware of safeguarding requirements and report any concerns Carry out duties with due regard to the St Lukes EDI Policy. Report any concerns regarding colleagues and staff members to prevent harm e.g. harassment/bullying/stress. Share knowledge, skills and expertise with colleagues and staff members.

Participate in and contribute to team meetings. Co-operate and liaise with departmental colleagues. Adhere to any financial responsibilities as aligned to the post. Ensure finances are used efficiently and in keeping with St Lukes charitable objectives.
