Our client is a leading care provider in the industry, committed to delivering high-quality care to individuals in need. They are seeking a Care Coordinator to work closely with their team, ensuring the smooth coordination of care services and the provision of excellent support to clients.

The Role:

  • Coordinate and schedule care services, ensuring that clients receive the appropriate care at the right time
  • Liaise with clients, their families, and care staff to assess care needs and develop customised care plans
  • Monitor the delivery of care to ensure it meets the quality standards and regulatory requirements
  • Maintain accurate records of client care plans, progress, and any changes in care requirements
  • Provide support and guidance to care staff, ensuring they have the necessary resources and information to deliver exceptional care
  • Collaborate with healthcare professionals, community organisations, and other stakeholders to enhance the overall care experience

Requirements:

  • Previous experience in care coordination or a related role
  • Excellent organisational skills with the ability to prioritise tasks and meet deadlines
  • Strong communication and interpersonal skills to effectively interact with clients, families, and care staff
  • Knowledge of care regulations and industry standards
  • Ability to problem-solve and adapt to changing situations
  • Proficiency in using computer systems and software for record-keeping and scheduling
