Job Summary:

Hire Staff is seeking a highly skilled and compassionate Registered Nurse to join our team as a Patient Care Specialist. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality patient care and ensuring the well-being of our residents.

Main Responsibilities:

  • Coordinating Patient Care: You will be responsible for coordinating, assessing, planning, implementing, and evaluating individual patient programs of care, with a focus on risk assessment and risk management strategies.
  • Supervising Staff: You will ensure that all staff contribute to the efficient running of the home and create an atmosphere conducive to the best interests of the residents.
  • Reporting Ill Health: You will report any ill health amongst residents and make requests for GP visits where necessary.
  • Empowering Residents: You will support the empowerment of residents, having regard for their dignity, rights, and advice, while maintaining professional responsibility for individuals in our care.
  • Care Planning: You will ensure that individual care plans are completed and maintained in conjunction with residents, relatives, and other healthcare professionals, and in accordance with NMC guidelines.
  • Therapeutic Interventions: You will participate in the planning, delivery, and evaluation of specific therapeutic interventions.
  • Health Supervision: You will assess residents in all aspects of their care needs to provide health supervision and attention through direct nursing care when needed.
  • Social and Health Care Needs: You will assist with the social and health care needs of new residents, maintaining ongoing assessment and review of all other patients.
  • Regular Checks: You will carry out regular checks on residents at intervals determined by the Matron.
  • Active Care and Rehabilitation: You will support the development of the process and practices of active care, recovery, and rehabilitation.
  • Medicine Administration: You will administer prescribed medicines and document the same in accordance with The Geoghegan Group procedure and Healthcare Commission guidelines.
