We are looking to recruit an innovative, enthusiastic band 7 Nurse. This is a truly exciting time to join us as we forge even strong links/ relationships across our hospitals and community to further enhance our services. Enabling people to receive rehabilitation and support to regain their independence or plan for their future care needs.

We are an enthusiastic, motivated multi-disciplinary team working in a dynamic, exciting environment, providing both step down service to facilitate discharge from hospital and step up service to avoid unnecessary admission to hospital. Embracing these models, we support our Crisis Response teams, city wide hospitals and neighbourhood integrated teams, to ensure that people can access our services when needed.

Underpinning the purpose and approach of our Intermediate care service we support patient's personal choices, strengths, and ensures inclusion in every stage of their care.

We work with our Manchester control room and city- wide teams to promote timely admissions to Intermediate care, aiming to reduce acquired hospital deconditioning, enabling a person to maintain as much of their independence as possible.

Main duties of the job

We are seeking a new team member who shares our passion for supporting people requiring Intermediate Care services and our enthusiasm for making a difference.

The service has both beds in a dedicated Intermediate Care unit and is also resourced to deliver Intermediate Care to 60 - 70 people in their own homes. All staff rotate with in the service, so it is essential that applicants can travel independently.

Clinical Practitioner and so will need excellent clinical assessment skills and will have (or be willing to undertake) Non-Medical Prescribing.

You will work within our multi-disciplinary team and liaise with our colleagues in the acute, community and voluntary partners to ensure admissions into the service and discharges are safe and effective and prevent re-admission.

We pride ourselves on promoting quality improvement initiatives, service development and professional and personal development and as a member of our team you will receive regular supervision and in-service training. You will be involved in both team projects and wider trust initiatives giving you the opportunity to form close professional relationships with a wide variety of multi-disciplinary team members.

You need to have experience of working in the NHS and possess excellent leadership skills. It would be desirable that you have experience of working in a rehabilitation/ community setting.

Qualifications

Essential


  • NMC Registration
  • Degree/Diploma in nursing

Desirable


  • Relevant Post graduate Training
  • Clinical Examination Skills. OCSE Level accredited course.
  • Registered independent prescriber (V300) or working towards this

Experience

Essential


  • Substantial post registration NHS experience
  • Broad experience at Band 6 working in community setting / Rapid Response / Intermediate care
  • Experienced in providing clinical leadership and problem solving.
  • Understanding of complex medical, social, and psychological needs of older people

Desirable


  • Evidence of involvement in change management

Knowledge

Essential


  • Knowledge of rehabilitation of older people.
  • Knowledge and understanding of complex medical, social and psychological needs of older adults.
  • Knowledge of managing difficult situations and conflict resolution.

Desirable


  • Excellent knowledge of current legislation and protocols within Intermediate Care / Crisis response
  • Previous experience at Senior level
