An exciting opportunity has arisen to recruit a Band 7 Discharge Coordinator in the Discharge Team to complement our nursing and admin team.
You will be working as part of a multi-professional Team to support the smooth running and co-ordination of discharge services within the unplanned Care Division and preventing unnecessary, avoidable delays for patient discharge.
The post holder will be responsible for facilitating safe and effective discharges of patients from hospital to the community setting through a multidisciplinary approach over 7 days. The post holder will work closely with all adult ward teams ensuring appropriate referrals are completed appropriately and made to local authorities.
