Specialist Community Public Health Nurse (Health Visiting) - Band 6- Wandsworth

Central London Community Health Care Trust (CLCH) has exciting opportunities for passionate and committed Health Visitors to join us in Wandsworth. We are looking for candidates with excellent clinical and communication skills to join our 0-19 teams contributing to the delivery of the Healthy Child Programme. In our last CQC inspection CLCH was awarded a "GOOD" rating, we are listed amongst the Top 120 NHS organizations to work for by HSJ. We consistently achieve our KPI's and work closely with the local GPs. We have strong links with our colleagues in Early Years and the Local Authority. Our teams cover Tooting, Balham, Earlsfield, Battersea, Putney, Southfields and Roehampton. We have a dedicated health visiting team for homeless families who are temporarily residing in local authority accommodation.

CLCH welcomes applications from newly qualified and experienced Health Visitors.

Main duties of the job

Car drivers are desirable but not essential.

You must have SCPHN - Health Visiting (BSc course) qualification and RGN Registered Nurse (Adult or Paediatric) or Registered Midwife.

From April 2023 a Recruitment premia has been introduced to attract and retain qualified Health Visitors within CLCH's Health Visiting Teams.

For registered Band 6 Health Visitors, this post is offering a premium of £2,500 payable upon commencement of employment (T&C's apply)

Job description

Job responsibilities

Deliver the healthy child programme and work in partnership with families to develop and agree tailored health plans as required

Utilising specialist skills and knowledge, advise and support individuals and families on evidence based health care practices

Understand the caseload profile including the health needs of the defined population and local communities, targeting outreach services to meet their needs

Initiate referrals to multi-disciplinary/agency colleagues as required to ensure adequate care and support for families and children

Assess, plan, implement and evaluate individual/group programmes of care that meet the public health needs of the population

To develop and delegate appropriately the implementation of regular health promotion initiatives on specific health issues and to specific groups and individuals

Education/Qualification

Essential

Registered Nurse/Registered Midwife Registered Health Visitor (SCPHN) Current NMC registration Evidence of continuing post-registration education and training Practice assessor/supervisor training or working towards

Experience

Essential

Health visiting experience Be able to co-ordinate health promotion initiatives in line with local and national health improvement targets Experience of working collaboratively as part of a multidisciplinary team and inter-agency working Experience of organising and managing own workload and that of others Experience of assessing, planning and implementing programmes of care and writing care plans Experience in providing practice based learning support to others Experience of evidence based care ? Experience of supervising staff / students

Skills and Knowledge

Knowledge of recent developments in primary care Knowledge of current health and social care agenda relevant to area of work and professional practice Knowledge of current health legislation [including Change for Children agenda, Children's Plan etc. Current knowledge of child protection issues Appropriate knowledge of clinical skills Knowledge of quality issues, the audit process and clinical governance Knowledge of NMC Code ?
