What does a Care Coordinator do?

Our Care Coordinators play a vital role, ensuring we deliver the best care to our clients. They do this by understanding the care arena, and knowing how to assess clients needs. They have excellent listening skills, are methodical and able to find solutions. They match carers to clients, and build up our contract base using their sales skills to upsell.

Key areas you might be dealing with;

· Managing new enquiries

· Dealing with rebooking’s and carer staffing changes

· Handling client request and proactively addressing their needs

· Speaking to MTD’s to ensure care plans are bespoke and high quality

· Responding to emergency situations, and supporting our of hours on call

Activities may include:

· Communicating effectively with the client and building strong lasting relationship

· Managing our carer workforce, giving them direction and feedback

· Working closely with the wider Draycott Head Office support team to ensure the best service for our clients

· Keeping accurate records and logging data in our systems

Experience

· 2 years plus experience

· 1 years experience of coordinating care in a domiciliary setting 

· Experience of growing contract hours, managing new enquiries

· Experience of high end clients/VIPs essential

· Experience of a range of clinical conditions

· Strong verbal and written communication skills for interacting with patients, families, and healthcare providers.

· Ability to manage multiple cases simultaneously

· Skills in coordinating with different healthcare professionals to provide holistic care.

· Ability to develop and implement solutions to improve patient outcomes.

· Experience in connecting patients with appropriate external support services.

*Must be willing and able to undertake an Enhanced DBS check as a mandatory requirement (or show proof of being current on the Update Service)

*Must be eligible to work in the UK without the requirement of company sponsorship
