We are currently recruiting for a **Care Coordinator** to maintain and develop our home care services.

**ACCOUNTABLE TO**: Senior Care Coordinator / Deputy Care Manager

**Core Duties & Responsibilities as Care Coordinator**:

- Direct Management of the care team monitoring staff development through supervisions, appraisals, and training to assist them in delivering a high-quality service
- Creating, managing, and monitoring customer care plans, ensuring staff rotas meet service users’ needs and aspirations
- Ensuring care assessments, care and risk planning meets best practice and customers aspirations
- Work with Recruitment Team to ensure sufficient current and future staffing levels
- To provide care services to service users in an emergency, arrange emergency cover when required and cover all/ any calls reasonable where required
- To ensure that the telephones are answered promptly, and people are spoken to in a polite and respectful manner
- To work alongside the Management/Team ensuring that all quality standards, complaints, and comments are dealt with promptly
- Participate in the on-call rota
- Put in place measure to comply with CQC regulations and work to achieve the best care standards possible
- Deploy and supervise team/carer when necessary
- Mentor new members of staff
- Support existing care staff in new skills
- Visit and provide a high standard of care in an efficient, kind, and respectful manner, preserving the dignity and self respect of the client at all times in emergency situations
- Maintain a smart and tidy appearance and always wear the appropriate clothing whilst on duty
- Complete a report for each client when taking lead in relation to setting up care package and all necessary follow up tasks.
- Attend/Lead training sessions and fire drills, as required
- Ensure confidentially and Data Protection is adhered and no matters concerning clients or business operational are not discussed with other clients or outside the office with anyone one else.
- Observance of Health and Safety requirements
- Report any accidents involving either staff or clients
- Comply with written and verbally agreed conditions and employment e.g. hours of duty, days of duty, time-keeping, timesheets etc.
- Be prepared to do extra or other duties on occasions as requested in line with the job

**We are looking for a Care Coordinator to join us who are**:

- Flexibility to cover on call to meet the needs of the business (essential)
- Knowledge of CQC standards and CQC Compliance (essential)
- Experience in Care Coordinating minimum of 1 year or 2 years working within Health & Social Care (preferred)
- Relevant qualifications in Health and Social Care (preferred)
- Basic skills requirement: including numeracy, literacy, and English-speaking abilities
- Knowledge and understanding of Dols/Mental Capacity Act/Safeguarding
- Preferably NVQ level 2 or similar qualifications
- Good written and oral communication skills including an ability to be sensitive to the needs of clients with mental health issues and an ability to work collaboratively with a wide range of people including Social Workers, practitioners, and other professionals.
- Good IT skills including proven experience of using Microsoft Office, particularly Word, Excel and PowerPoint

**Job Types**: Full-time, Fixed Term

Schedule:

- Monday to Friday: 9am-5pm (Including. out of hours mobile on rotational basis)

- Healthcare Industry: 1 year (required)
- Care Coordinating: 1 year (required)

Work Location: In person

**Salary**: £19,000.00-£25,000.00 per year
