You will hold an appropriate professional registration (NMC, HCPC etc) , will have a strong proven clinical knowledge within mental health and demonstrate ongoing professional development. Previous experience of working with older adults and adults of working age is desirable.


The role will include use of computer systems so proficient use of IT is necessary.


This is an exciting opportunity to have an impact on the quality of mental health care for patients requiring specialist assessment at the Great Western Hospital. By joining the Hospital Liaison Team based at the Victoria Centre in Swindon you will be providing assessment for people across the age range with mental health illness, including dementia, who are admitted to the Great Western Hospital (GWH).
The successful applicant will work within the specialist Older Adult Liaison Team. You will provide a responsive, needs-led mental health assessment and formulate and deliver short-term interventions for individuals presenting to the team with a range of mental health problems.


Referrals originate from a wide range of sources and high quality interdisciplinary communication is essential.


There will also be a teaching, education and support role, assisting non-mental health staff to respond to mental health needs more effectively in a range of settings.


This is a challenging, but rewarding post, within a small PLAN Accredited team who are extremely proud of the high quality, compassionate assessment and care they deliver in collaboration with service users their families and health and social care services.


The team facilitate placements for students from a range of health professions.

1 Be responsible for the full assessment of patients presenting withDementia and a range of undifferentiated mental health and psychologicalproblems. As a result of this assessment, you will be responsible for thedevelopment, implementation and evaluation of programmes of care,without direct supervision.4 Promote a positive attitude regarding mental health across the generalhospital and residential care home settings.


2 Undertake specialist psychosocial assessments with patients presenting with complex physical and mental health needs highly developed riskassessment and management skills are required in order to undertake thisfunction effectively.3 Prompt referral on to secondary mental health services, social servicesand third sector organisations as appropriate.4 Flexible, needs-based follow-up of individuals until ongoing carearrangements are agreed and in place2.6 Effective liaison and communication with patients, relatives, carers,primary health care and secondary mental health services.8 Role model clinical excellence and high standards of nursing practice toother staff.9 Initiate, participate in and oversee effective implementation andadministration of the Mental Health Act (1983), and the Mental Capacity Act (2005), including the use of Deprivation of Liberty Safeguards and BestInterest Decision making process


1 Following negotiation, take responsibility for agreed clinical andmanagerial projects associated with the provision of the service.2 Ensure the promotion of the health and safety, well-being and interests of patients, staff and visitors that come into contact with the service and itsenvirons.3 At all times adhere to the NMC Code of Professional Conduct.4 Supervise, monitor and appraise the performance of registered andunregistered staff within sphere of responsibility.1 Participate in the delivery of appropriate audit, evaluation projects andservice development (including data collection where appropriate).


2 Participate in the mentoring and education of a range of students onplacement.4 Participate in the planning, development and delivery of multi-professional education programmes and training sessions for a range of clinical and managerial staff.5 Provide clinical supervision to colleagues and students, as required.


Quality and Effectiveness


3 Adhere to relevant trust health and safety and clinical policies.

Evidence of professional updating and development in the nursing care of patients in an acute or community setting.

Experience of practicing in acute, community or rehabilitation mental health settings.
Experience of practicing in specialist dementia and older people's settings - either in patient, residential or community

Experience of practicing in a mental health liaison setting.


Knowledge of the various approaches to mental health assessment.
