We have an exciting opportunity for a Multi-Disciplinary Team Coordinator to join and help us deliver the best possible outcomes for children and young people with SEND.

As a Multi-Disciplinary Team Coordinator, you will assist the Specialist Practitioners and serve as a liaison between the iSEND Service and all internal and external professionals. You will be responsible for ensuring compliance with statutory regulations, developing and monitoring interventions as well as contributing to service development and improvement.

Contract: This is a fixed term post for the duration of one year.

About the role

In this role, you will have the opportunity to work alongside professionals within education, health and social care to manage and progress effective multi-disciplinary practices. You will be responsible for processing and reviewing Education, Health & Care Plans (EHC Plans), communicating with relevant organizations and stakeholders and ensuring compliance with statutory regulations and timescales.

Some of your day-to-day tasks will include:

  • Establishing and maintaining effective communication with internal and external organizations.
  • Developing a multi-agency person-centred approach for the children and young people.
  • Overseeing and collating evidence of children and young people’s progress.
  • Engaging in quality control management and ensuring compliance with statutory timescales.
About you

We are looking for a candidate who is passionate about making a difference in the lives of children and young people with SEND. You should have relevant experience and knowledge of the service area, legislation, SEND policy and process. You should also have good communication, interpersonal, organisational and ICT skills.

To be successful in this role, you will need to have the ability to work on your own initiative with complex issues, be flexible and able to work under pressure.
