Job description
Connect2Halton is seeking compassionate community support workers within Halton Borough Council. You will act as key worker to a number of children and families, working in partnership with them and any other relevant persons to identify needs and co-ordinate/implement the individual care plans. We are offering an hourly salary of £15.43ph.

Job Overview
- Contribute to the creation, implementation and review of individual care plans for children, young people and their families to meet their assessed need.
- Ensure that the personal care needs of the children and young people are met.
- Participate in the corporate administration of the service both within the Team/Centre and in the community.
- Maintain effective record systems to enable the preparation of necessary reports to contribute to the care planning process.
- Available to work out of hours to meet the demands of the service.
- Where appropriate, participate as a working supervisor of lower graded staff, to ensure the effective and efficient organisation of the service.
Experience/desirables
- Previous experience in a similar role is desirable.
- Excellent communication and interpersonal skills
- Must drive (with business insurance)
