As an Advanced Nurse Practitioner (ANP) you will play a pivotal role in triaging assessing diagnosing and managing patient care plans working collaboratively with General Practitioners and multidisciplinary teams. We offer a competitive salary professional development opportunities and a commitment to work-life balance in a vibrant community setting.Key ResponsibilitiesTriage assess diagnose prescribe and develop individualized management plans for patients with acute and chronic conditions.Manage care for residents in linked care homes addressing complex physical health conditions dementia mental health needs and challenging behaviors.Collaborate with General Practitioners pharmacists and other healthcare professionals to review cases provide advice and facilitate referrals for further investigations or services.Perform advanced clinical procedures such as cervical screening minor illness/injury management and chronic disease monitoring (e.g. diabetes care).Contribute to service improvement initiatives ensuring high standards of patient care and safety.Maintain accurate patient records and adhere to clinical governance and safeguarding protocols.Engage in continuous professional development to enhance clinical expertise and leadership skills.What Were Looking ForRegistered Nurse with current NMC registration.MSc or equivalent Advanced Nurse Practitioner qualification.Independent/Supplementary Prescriber qualification.Proven experience as an Advanced Nurse Practitioner in primary care community settings or care homes.Advanced clinical assessment and diagnostic skills with expertise in managing minor illnesses minor injuries and chronic conditions.Experience in complex wound care venipuncture cannulation and other advanced clinical procedures.Strong communication and interpersonal skills with the ability to work effectively within a multidisciplinary team.Commitment to ongoing professional development and maintaining up-to-date mandatory training (e.g. flu vaccination safeguarding and CPR).Right to work in the UK.What We OfferCompetitive salary with performance-related incentives.Generous support for continuous professional development including funded CPD days (up to 10 days per year).Comprehensive in-house training programs including mentorship and clinical leadership development.Access to wellbeing and lifestyle benefits including flexible working options.Opportunity to work in a supportive community-focused healthcare environment in Leighton Buzzard.Pension scheme and generous annual leave entitlement.Relocation support for candidates moving to the Leighton Buzzard area (if applicable).Additional InformationVacancy Type: Permanent Full-time or Part-time (flexible hours available)Positions Available: 1Application Process: Shortlisted candidates will be contacted for an interview.
