Salary: NJC 9 - NJC 11 £33,945 to £44,428 depending on experience
Working Pattern: Part time, 23 hours per week
Contract Length: Permanent
Location: Chichester, Bognor, Worthing
The Transition Teams are county-wide teams that offer a service to young people from the age of 16 who have lifelong disabilities. Lifelong disability transition services are for those young people with physical disability,learning disability and / or autism. The focus of the team is to support people moving from childhood to adulthood and plan for the time after the person turns 18.
As a Social Workerwithin a busy community team,you will be responsible for achieving positive change and improved outcomesfor young people. You will have avaried, complex caseload,and provideanaccountable social care service to those people who access or need the service. You will be undertaking high quality assessments and reviews including mentalcapacity act assessments, responding to safeguarding enquiries,andworking as part of a team. You will beexpected to work within organisational, policy and legislative requirements.You will have the opportunity to apply your social work skills, experience and values to the work that you do in supporting young people, their support networks and your team.Hear from the team:
