Are you looking for an exciting new challenge? We are looking for a Duty Worker /Progress Chaser to join the Housing Solutions team. This a vital role in the daily running of our team and supporting citizens of Hertfordshire to access essential adaptations to their homes enabling them to live as independently and safely as possible.

The Connected Lives Gateway Service is our front door for Hertfordshire residents seeking information and support around adult social care. We aim to prevent, reduce and delay need by ensuring people access a wide range of information, advice, and preventive and enablement support in a straightforward and timely way. We are looking for friendly, enthusiastic & committed people to join us ensuring residents access a wide range of information, via our proportionate interventions, and receive support at the right time and in the most uncomplicated way possible.

Working within the Gateway Service, the Housing Solutions team offer specialist assessment across Hertfordshire for major housing adaptations. The team has a blend of community based Occupational Therapists and Community Care Officer Trusted Assessors who complete specialist housing needs assessments to identify major adaptation solutions to enable a person to have better access to the facilities within their homes. We explore personal strengths, their family and friends’ support networks, and using expert knowledge we connect people with things that will help individuals and families to get on with their lives, achieving prevention, reducing or delaying the need for statutory provision of care services.

You will work as part of the Housing Solutions team and also collaboratively with other partners and teams, both internally within Adult Care Services and externally with the Hertfordshire Home Improvement Agency, District and Borough Councils and Housing Associations.

Your role will include supporting the Housing Solutions management team by completing duty tasks including contacting citizens by telephone to obtain further information regarding their referral and issues they require our help with. You will liaise regularly with Housing Associations and District Council adaptation teams in order to progress the recommendations made and play an essential role in resolving concerns that arise. You will act as an important link between housing providers, citizens and the team, working to achieve efficient and prompt communication channels. You will use our record system to monitor and progress chase adaptation requests and promptly document the actions taken, updating the record system appropriately.

You will need to be a confident individual, a good team player and an adaptable person. The role requires you to be an effective communicator, a problem solver and have attention to detail. You will need to be well organised and posses effective time management skills to manage competing demands on your time and have the ability to prioritise tasks appropriately. We need you to be able to work successfully with partners and many other agencies for the benefit of citizens. Your self-organisation is key, as is a ‘can do’ attitude.
