Principal Duties and Responsibilities

The Care Coordinator will be responsible for ensuring high quality care and support within the services are provided to enable individuals to remain in their own homes, living a fulfilled life, which respects their diversity, culture, and values.

Responsibilities of the Care Coordinator is to support the delivery of the highest quality care support services in a hands-on way. Integrating the SPECAL method into daily practice is essential, alongside upholding client dignity and autonomy. Promptly reporting any changes or concerns to management is crucial. Knowing when to seek assistance and guidance to ensure client safety and well-being is paramount.

Duties of the role will include (but are not limited to) to following:


• Managing the distribution of Care & Support Workers responding to service delivery issues as they arise.


• Assisting with the induction and training of Care & Support Workers within the company including SPECAL Training.


• Implementing, monitoring and evaluating the Individual Support and Assessment Procedures within your area of responsibility.


• Communicating with customers, their families and other care professionals.

Requirements


• A diploma in Health & Social Care Level 2 or 3


• Dementia awareness certification at level 2


• Previous experience working as a Care Coordinator, as a supervisor or other role within a care providing organisation.


• Previous Dementia care experience in a paid or unpaid role is essential.


• Proficiency in the English language, together with excellent communication and written skills.


• An understanding of The SPECAL Method (Specialized Early Care for Alzheimer), as the successful candidate will undertake full SPECAL training provided by Care 24-7 on acceptance of the job role.


• A strong understanding of legislation concerns with care provision.


• Enhanced Disclosure from the Disclosure and Barring Service.


• A full UK drivers' licence and access to own transport.


• Right to work in the UK.
