We have an exciting opportunity for a Care Navigator to work seamlessly with the other Care Navigators and Apprentice Health and Wellbeing Coaches, to ensure holistic and meaningful support is provided to those with health vulnerabilities on their release from prison, ensuring continuity of care and fruition of release plans.

Care Navigators use a coaching framework, to manage wellbeing in the pre and post release period, to prevent a return to ill health as well as a reduction in reoffending. By preventing this decline in health, through robust reconnection with health services in the community we will reduce health inequalities for those leaving the criminal justice system and provide support to allow a seamless, and effective transition to independent community living.

You will work with the following people and teams:

Head of Prisons and Criminal Justice

Head of Care Navigation

Contract Manager

IAPT Clinical Leads

Health and Wellbeing Coaches (Probation Services)

Care Navigators

Apprentice Health and Wellbeing Coaches

The successful candidate will have:

  • Educated to NVQ Level 3 or equivalent with qualifications or training relevant to the Care Navigation role, e.g. Health and Social Care
  • Relevant experience of supporting vulnerable individuals to identify, set and meet their own goals using 1:1 and/or group interventions
  • Working knowledge of health, debts, social care, housing, court systems, mental health, criminal justice and substance misuses
  • Experience of working to support individuals using structured, time limited support and am confident with this way of working
  • The passion and motivation to support individuals leaving the criminal justice system through provision of a high quality, professional level of care, recognizing and respecting individual differences and promoting diversity
  • A full UK driving license and have access to own transport for work purposes
  • Excellent time management skills, understanding the need to work effectively and efficiently at all times
  • The ability to develop professional relationships with patients and colleagues, and practice in a non-discriminatory manner, respecting the beliefs and cultural practices of individuals and groups.
  • Confidence in communicating and have excellent verbal and written skills.
  • Working knowledge of standard IT packages including Microsoft Word, Excel and use of database / information systems.

A Driving License and access to own Transport is Essential for This Role*
