About the Role

We are seeking a highly skilled and compassionate individual to join our team as a Mental Health Navigator at Birmingham Mind. As a key member of our Acute and Urgent Care team, you will play a vital role in supporting individuals with mental health needs to achieve their recovery goals.

Key Responsibilities
  • Work closely with individuals, families, and carers to develop and implement personalized care plans
  • Coordinate discharge planning to ensure a smooth transition from hospital to community-based care
  • Collaborate with healthcare professionals to provide comprehensive support and guidance
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed
Requirements
  • Strong understanding of mental health principles and practices
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Highly organized and able to prioritize tasks effectively
