About the Role

We are seeking a skilled and compassionate Discharge Navigator to join our team at Birmingham Mind. As a key member of our acute services team, you will play a vital role in supporting individuals to achieve a smooth transition back into their community.

Key Responsibilities

  • Work closely with our Discharge Manager to prioritize and manage a caseload of individuals on a recovery journey.
  • Participate in bed management and multi-disciplinary team meetings to ensure effective care planning.
  • Collaborate with statutory and third sector providers to identify and secure appropriate housing and community support networks.
  • Develop and maintain strong relationships with individuals, families, and carers to promote recovery and well-being.

Requirements

  • Previous experience of working in a paid role in the mental health field, with a focus on person-centered support.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Organizational skills, with the ability to prioritize and manage a caseload.
  • A full UK driving license and use of a vehicle (desirable).
