Lead Practitioner

We are delighted to offer this recruitment opportunity for a Lead Practitioner within the Sleaford/Spalding Learning disability team. The post is full time and a permanent position.

The Lead Practitioner post is both an exciting and challenging role. For those who love fast paced and 'make a difference' work, this is the position for you. You will need to hold either a Social Work or Nurse qualification and be able to show your registration with SWE/NMC is in date. The nurse qualification does not need to be specific to Learning Disability.

  • Ensure your team are working within the financial constraints of our service budget, arranging strength-based cost-effective packages of support
  • Work in partnership with other agencies and stakeholders and where needed arrange and chair multi-disciplinary and best interest meetings
  • Possess a Comprehensive knowledge and understanding of the legislation that underpins Adult Care.
  • Safeguarding knowledge must be up to date, and you should be fully familiar with your own and team's responsibilities.
  • Put systems and processes in place to monitor and review your team are on track with their key performance indicators
  • Be able to competently manage any HR issues

    About the Role

    You will manage a team of qualified and unqualified Practitioners. Your team will have an allocated case load which includes complex and high-risk cases. Your team will carry out strength based Adult Needs Assessments and Reviews, working with service users and their families to maximise independence, assessing, and managing identified risks, taking into consideration any informal support provided.

    We work in accordance with the Care Act 2014, we are creative to ensure needs are met. For example, support in the community, voluntary services, or use of equipment such as Telecare. We must ensure the public purse is managed wisely.

    About the Team

    The Learning Disability team take great pride in doing their job well and are passionate about providing the best service they can for the people who access our specialist service.

    Our dedicated team consists of a Lead Practitioner, Qualified Practitioners (Nurses and Social Workers) and Community Care Officers. Our Practitioners possess a wealth of knowledge and skills, they are supportive and nurturing with each member bringing their own skills/qualities and uniqueness to the team.

    The post will be based at either Sleaford – 105 Eastgate, Sleaford NG34 7EN, or Spalding: The Vista, Churchgate, Spalding PE11 2RA
