The Broxtowe Living Well Team are looking for a full-time Community Care Officer to work with adults aged 18-65 with a physical disability. This is an exciting opportunity to work in a strength-based way with people, to enable them to maintain independence.The role involves visiting people in their own homes and assessing eligibility under The Care Act, as well as small aids and equipment to enable independence. You will also provide support to carers and undertake adult cares assessment, as well as young carers assessments. In addition you will undertake written assessments in order to commission support through homecare agencies and direct payments.You will need to have:
  • At least two years experience through paid employment in a social care setting or extensive personal experience or voluntary work.
  • Knowledge of the following: the NHS and Community Care Act, Mental Capacity Act, Safeguarding Adults and Continuing Health care.
  • Good communication, time management and IT skills are essential.
