The UHL Discharge Team facilitate the discharges and transfers of patients in the acute setting with new and increased care needs, back into the community via the appropriate discharge pathway


We are looking for a Discharge administrator to be part of a small team based in the LLR discharge Hub.

You will need comprehensive knowledge of clerical and administrative systems to help facilitate discharges. You will provide support to members of the multi disciplinary team.

  • To support and advise and work closely with staff and patient relatives ,from other departments and Clinical Management Groups across the Trust as required.
Tailoring junior staff teaching, adapting it to suitable levels. Liaising with external partners.

To support safe, effective, and timely Discharge of patients to their appropriate destination.

  • Processing Home first forms from internal systems, transferring them external systems for processing.
  • To demonstrate the Trust's values in everything you do in the work environment.
  • To be responsible in the use and expenditure of the Trust's resources that you utilise.
  • To act independently within the team processes and guidelines deciding when it is necessary to refer to the escalate to Discharge Matron.
  • To prioritise workload to meet service demand.
  • Registering patients onto databases and supporting progression of their discharge.
  • Creating and cleansing of Nerve Centre and S1 data base for senior partners to inform Discharge Hub performance and future commissioning.
  • Pull data/reading/ producing data.
- liaise with System Partners, carers, family members and MDT to communicate safe and timely discharge to ensure they receive correct verbal and written information about discharge via the appropriate discharge pathway.

  • To ensure face to face and virtual queries are dealt with swiftly and appropriately, and
support is sought where necessary.

  • To ensure that handover information and the escalation of urgent tasks occur in real time.
  • To collate data to monitor the effectiveness of service provision.
To ensure reports are collated and submitted to senior management within agreed timeframes.

  • To work in line with policies and procedures relevant to patient discharge and information governance.
  • Practice in a punctual, dynamic, and organised manner to ensure the needs of the service are met, whilst always considering the boundaries of your role.
  • To arrange for appropriate information to be shared with the care providers for safe handover of patient / right external partner.
