Ealing Intermediate Care service has an exciting role within the local management team for an enthusiastic and motivated Modern Matron experience of working within a multiple disciplinary team at a senior level to provide daily operational leadership in line with the agreed service.
Your main role will be to provide sound clinical and operational support. Main duties of the job- To provide clinically excellent leadership across IC and develop peer network across ECP, WLT and CNWL.
- To contribute to development of the therapy pathways across IC, ECP and NWL.
- To ensure the successful implementation and roll out of the therapy pathway and the Quality Improvement Programme.
- To work closely with the MDT in providing seamless care across departments and professions
- To be responsible and accountable for patient flow and service delivery across the therapy pathway
- Hold and manage therapy ward budgets.
- Registered RG/MN
- Leadership qualification
- Post Grad diploma in leadership/service development
- Accredited Advanced Practitioner
- Experience of managing and leading services through transitional periods
- Experience of managing a team that incorporates multiple providers including ASC
- Experience of leading on discharge and admission pathways
- Managing a Nursing, Physiotherapy and Occupational therapy team within an inpatient or community setting
- Experience of leading on QSIR/quality improvement projects
- Experience of providing clinical interventions within an acute or community setting
- Experience of managing and monitoring performance across multiple teams
- Experience of developing new teams, pathway and ways of working
- Experience of winter resilience planning with commissioners and partner organisations
- Experience of teaching Nurses, AHP and Medical staff groups
- Experience of recruitment across multiple teams
- Clinical experience in discharge pathways, general & neurological rehabilitation services.
- Managing social care/reablement services
- Experience of managing intermediate care teams including rapid response, short term rehab, reablement and community beds
- Clinical experience in discharge pathways, general & neurological rehabilitation services.
- Understanding of Different admission pathways and future plan
- Knowledge of intermediate care guidance and best practice
- Working knowledge of NHS 10 year plan
- Broad medical knowledge including end of life, chronic conditions and frailty
- Knowledge of frailty models of care and comprehensive geriatric assessments and how to implement standards within a inpatient settings
- Knowledge of local commissioning objectives and 10 year plan
- In-depth understanding of intermediate care remit and agenda
- Knowledge of NWL intermediate care agenda
- Able to work seamlessly with other professional groups
- Communicate clear message and guidance
- Able to challenge practice constructively to implement change
- Remain professional at all times in line with NHS KSF
