We are seeking a Social Prescriber to join our practice of 9400+ patients.

15 hours per week and must be available for afternoon shifts. This role requires 1 day in practice and 1 day working from home.

Bridge House Medical Practice is open between the hours of 08:00 to 18:30 Monday to Friday. The successful candidate will need to be hard-working, flexible and enthusiastic. We are looking for a capable individual who is able to multi-task and work well under pressure.

Main duties of the job

As the Social Prescriber, you will provide followon support to service users from the assessment and care plan, or frominteractions with other professionals such as the GP. You will provide holistic assessments and co-designsocial prescription to improve health and well-being outcomes of individuals. In particular, you will:

1. Focus on helping patients to make full use of localcommunity and voluntary sector services available locally, as well as thoseprovided by health and social care providers.

2. Manage a caseload of patients through holistic assessmentto onward referral by providing ongoing support for a short-term timeframe topromote engagement with identified services and achievement of targets.

3. Update fellow teammates and members to increasetheir awareness and use of local services, develop new opportunities, andproblem solve to improve access.

PrincipalTasks

Servicedelivery

To take referrals from GPs and othermembers of the team for individual patients.

To discuss with the person theirneeds, based on GP guidance, and to direct them to appropriate socialprescription servicers.

To manage a caseload of clientsthrough holistic assessment to onward referral, working with clients in thepractice as identified by the GP. This will include provision of ongoingsupport for an allocated but short-term timeframe to promote engagement withidentified services and achievement of goals.

To develop knowledge of local services,using existing databases and developing links with service providers, keepingup-to-date with service changes and developments.

To inform and develop GPs and otherhealthcare professionals on how to identify patients suitable for socialprescribing service referral. The aim is to support the development of careplanning and case management to support patients in their management of careand avoid unnecessary use of primary care and hospital services.

To actively participate in practicelevel multi-disciplinary team meetings.

To work in partnership withall voluntary and community organisations to build a comprehensive database oflocal resources to design and support the Social Prescription activities.

To train and develop GPs and primary care health teams knowledge on how to identifypatients suitable for social prescribing service referral on a quarterlybasis.

To set up and maintaincomprehensive data and evaluation systems, including individual health outcometool (such as Recovery Star).

Leadershipand management of people

Tosupport and supervise volunteers (as assigned) to ensure performance targetsare met.

Tolead by example, upholding Bridge House Medical Practice codes of conduct,policies and working practices.

Toincrease the participation and involvement of residents in the activities of local services and other practice activities.

Wider Partnership working

Tocontribute to initiatives to develop partnershipsincluding developing and maintaining effective working relationshipswith local residents, voluntary and community groups,statutory and public sector organisations, businesses and funding bodies.

Towork collaboratively with internal and external partners to identify and securefunding streams and resources to support delivery of the Practice objectives.

Team working

Totake part in all practice events andactivities as agreed with your line manager.

Topromote a positive team environment and work well as part of the Trust staffteam to co-ordinate activities andresources
