We are looking for six well motivated and enthusiastic people to work in our Healthy Hearts Programme for two years fixed term.

Working across Carmarthenshire, we need 2 people in each of our localities Llanelli, Amman Gwendraeth 2T's area.


The post holder will work in a variety of Community and Primary Care settings working within the clusters as part of the countywide CVD Risk Assessment project.

The post holder will conduct cardiovascular disease risk assessment health checks, supported with a customised computer software programme, on residents invited to the programme from their GP practices.

Work with local companies to undertake risk assessment health checks for their staff.

The post holder will be expected to undertake appropriate training to use the software and also to develop excellent communication and behaviour change skills which will enable them to support and motivate patients to make positive lifestyle changes, identifying their own CVD risk factors and enable them to reduce and manage their risk.


Following training, use risk assessment computer software to deliver the HDUHB Health Check programme, and to ensure accurate data entry and regular synchronization of health check records.

Perform or undertake appropriate training in order to perform the following key skills:
Weight, Height, Waist circumference
Venepuncture
BP monitoring
Point of Care testing

Advise on healthy living options and behaviours for individuals, in their context.


Help individuals to develop an 'action plan' to change behaviour and sustain those behaviours, leading to more healthy choices and actions.

Support individuals' 'action plan' over an agreed time through practical help.
Review and revise 'action plans' with individuals as appropriate.
Help individuals to identify and access local services.
Keep agreed records and monitor progress.

Have a good knowledge of local services which can support clients in improving their health and wellbeing and reduce their CVD risk.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
