We have an exciting opportunity for you to join the Outreach Service to help facilitate hospital discharges.

We are looking for motivated, enthusiastic people who have the ability to work well within a team.

The post holders will be responsible for providing support for patients in their own home environment following discharge from hospital and building confidence to be able to undertake their Activities of Daily Living.

You must have excellent communication and interpersonal skills to enable a positive patient experience.

The post holder will work a range of shifts over a seven-day period including bank holidays to meet the needs of the evolving service.

Supporting patients in their own home with personal care, meal preparation, medications, prevention of pressure damage and observations. Using EMIS to document patients care episodes. liaising with other health and social care professionals to meet the patients care needs.

The Trust provides services to the 440,000 residents of Blackpool, Fylde & Wyre and North Lancashire, as well as specialist tertiary care for Cardiac and Haematology patients.

Further afield, Blackpool also benefits from less than three-hour connections via rail to Edinburgh in the north and London in the south.


  • To ensure care and support is delivered to the patient both effectively and efficiently and communications are maintained within the parameters of care.
  • Provide reablement activities and care support to allow the service user to adapt back to their home environment.
  • Be able to motivate service user to maximise patient independence.
  • To prioritise own allocated workload and deal with daily changes using own initiative.
  • Provide support and encouragement for patients in undertaking activities of daily living in their home environment.
  • Promote excellent care standards aligned to activities of daily living that have been identified as part of the plan of care.
  • Provide care for clients with dignity and respect and be nonjudgemental in your approach to client care and attitude.
  • Build relationships with patients, their families and carers to enable optimal functioning of activities of daily living.
  • Undertake environmental risk assessments to reduce the risk of patient harms.
  • Support patients with selfadministration of medication to increase concordance.
  • To provide basic clinical care tasks, as directed by Team Leader or care plan, but not under direct supervision, e.g. medication prompting or assisting, catheter care and patient observations.
  • Work in partnership and collaboratively with other health, social care and voluntary services in hospital and community settings to ensure holistic and high quality care is provided.
  • To assist with safe moving and handling of patients in their own homes using equipment where necessary.
  • To record and maintain adequate records using computer based programme and written notes in line with Trust guidelines.
  • Give advice and support to patients, families and carers within duties appropriate to grade and level of competence.
  • To respond to emergency situations in patients own home and report back to the appropriate health care professional.
  • To observe, report and record changes in patient's condition at team handover.
  • Completion of relevant referral forms.
  • Promote healthy lifestyle to patients, families and their carers.
  • Adhere to Trust policies and procedures and keep up to date with changes within the role of Outreach Support Worker.
  • Instil eye drops both single and multiple and support patients to selfadminister.
  • To observe Trust policies and procedures in respect of: Child Protection; Data Protection Act; Care and Treatment of Pressure Sores and Safeguarding Adults.
  • Record relevant details of accidents and untoward incidents in accordance with Trust Policy.
  • Develop and maintain effective relationships with other members of the Outreach Team and stakeholders.
  • Keep up to date with all relevant mandatory and statutory training.
  • To attend staff meetings as required, including working in groups to direct and implement changes where appropriate.
  • To participate in audit as per audit calendar.
  • Embrace the Trust appraisal system and identify personal and educational requirements in conjunction with the manager.
  • Expand the role to fulfil the needs of the service when required.
