Huddersfield, Kirklees, United Kingdom Calderdale and Huddersfield NHS Foundation Trust Full time

Job overview:

The postholder will work with the Orthopaedic Consultant team to carry continuing responsibility for a defined caseload of outpatients and inpatients requiring specialist Orthopaedic intervention following an injury.

The post holder will work with the Orthopaedic Consultant to assess the care needs of individuals and ensure that a personalised plan of care is developed, implemented and evaluated.

Main duties of the job:

The postholder will provide nursing advice specific to the patient group or specialty and will work collaboratively with members of the Multidisciplinary team to ensure that best practice care is delivered and meets the needs of patients and their families.

Working for your organisation CHFT is at the forefront of the digital health revolution; with the launch of a full Electronic Patient Record (EPR) in 2017 the Trust started on its journey to be the safest, most efficient and patient-centred trust in the NHS.

EPR is part of an ongoing project to develop a digital ecosystem within the Trust to become a truly paper-free organisation as professional development.

Calderdale and Huddersfield NHS Foundation Trust is an exceptional place to work with many amazing teams of hard working dedicated staff, delivering high standards of patient care.

We have made significant changes to the way we work to meet the challenges of the changing NHS and we were rated by the Care Quality Commission as Good in June 2018.

We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients' homes.

We also are incredibly proud to have almost 150 volunteers here at CHFT.

We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children's and young people's services; end of life care and outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

We continue to modernise and invest in our health services to build on our strong reputation. Foundation trusts are public leaders in improving quality in health services.

They are part of the NHS - yet decisions about what they do and how they do it are driven by independent boards.

Boards listen to their Council of Governors and respond to the needs of their members - patients, staff and the local community.

Foundation trusts provide what the health service wants, yet are also free to invest quickly in the changes to the local community needs, in striving to be the best, and in putting their patients first.

Detailed job description and main responsibilities:

The post holder will contribute to the development of the specialty service as well as contributing to the strategic direction of nursing within the Trust.

The post holder will plan and oversee the delivery of specialist competency frameworks to ensure that registered nurses working in Orthopaedic Outpatients have the skills to be able to care for patients in a virtual fracture clinic setting.

The post holder will be responsible for ensuring that evidence based virtual fracture clinic pathways are developed and reviewed in line with best practice and national developments of this service model.
