Background


Social prescribing empowers people to take control of their health and wellbeing supported by a 'link worker' who can give time, focus on what matters to the individual and who takes a holistic approach to their health and wellbeing, connecting people to a range of diverse community groups and statutory services for practical and emotional support.


Social prescribing can help strengthen community and personal resilience, reduce health inequalities (in relation to timely access and outcomes) and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people's active involvement with their local communities.

It particularly works for people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.

Social prescribing is a way of engaging patients with a resource which provides support within the local community.

In addition it provides GPs and other healthcare professionals with a non-medical referral option that can align to existing treatments to improve health and wellbeing.

People want to be able to access information and support in a setting that is convenient and familiar to them, delivered by people they trust.


The social prescribing link worker will work as a key part of the Brompton Primary Care Network team working with the Royal Hospital Chelsea (RHC) to offer to Chelsea Pensioners and Pensioners who served in HM Armed Forces and are resident in Brompton PCN and registered with practices there.

Brompton PCN is the largest of five PCNs in West London with a population of around 102,000 including a substantial number of veterans


The social prescribing link worker will provide support in a clinic environment based within the RHC general practice, at the RHC Pensioner Outreach drop in sessions and where appropriate the patient's home.


Key responsibilities

  • Receiving and actioning referrals from a wide range of agencies, working closely with the Royal Hospital Chelsea GP practice, the RHC Pensioner Outreach service, GP practices in the local PCN, pharmacies, multidisciplinary teams, hospital discharge teams, allied health professionals, fire service, police, job centres, social care services, housing associations, and voluntary, community and social enterprise (VCSE) organisations.   • Provide personalised support to Pensioners who have served in HM Armed Forces and their carers to take control of their health and wellbeing, live independently and improve their health access and outcomes, as a key member of the PCN multidisciplinary team.
  • Develop trusting relationships by those individuals time and focus on what matters to them. Taking an holistic approach, based on the person's priorities and the wider determinants of health. Coproducing a simple personalised care and support plan to improve health and wellbeing, introducing or reconnecting people to appropriate community groups and statutory services.
  • The role will provide information and support to patients in addition to becoming the link between the individual, the RHC, the GP and other service providers. The role will require managing and prioritising a caseload, in accordance with the needs, priorities and any urgent support required by individuals.


It is vital that the Social Prescribing Link Worker has a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals or agencies.


Service delivery

  • Understand the Royal Hospital Chelsea contribution to HM Armed Forces Pensioners and the service it provides.
  • Build a robust knowledge of the health, social and third sector provision available within Brompton Primary Care Network
  • Promote social prescribing, its role in selfmanagement, and the wider determinants of health.
  • Act as an advocate for patients and service users of the health and social care system.
  • Build relationships with key staff at the RHC and with the GP practices within the local Primary Care Network (PCN). Attend relevant meetings and integrate as part of the wider network team, providing information and feedback on social prescribing matters.
  • Work proactively to develop strong links with all local agencies to encourage referrals, to recognise their requirements and enable confident approach to making referrals.
  • Work in partnership with all local agencies to raise awareness of social prescribing and demonstrate how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care.
  • Work proactively in encouraging selfreferrals and connecting with all local communities, particularly those communities that statutory agencies may find hard to reach.
  • To support patients on discharge from hospital admission.
