CLCH is one of the UK's largest NHS Trusts and an opportunity has become available to work for the Harrow Short Term and inpatient rehabilitation team as a Band 4 Rehab Support Worker.

We are currently looking to recruit an experienced Support Worker that is passionate about providing high levels of care and support to individuals with provide rehabilitation support to patients.


The post will have the opportunity to work in an integrated care setting working alongside physiotherapists and occupational therapists and supporting the team in the delivery of care to the Therapy caseload.

The post holder will work individually with patients supporting them in achieving therapy goals through improving functional ability, carrying out exercise programmes with patients and working collaboratively with the physiotherapists and occupational therapists in helping patient goals be achieved.

The post holder will be given support and training to broaden skills and progress knowledge. As well as a chance to work with different people in different areas within the service.


As a Rehabilitation Support Worker, you will be involved in meeting the therapy needs of a diverse group of service users with varying degrees of mental and physical issues.

You will be caring and attentive with the skills to build strong and trusting relationships.


You will also work well in a team as you will be required to work alongside qualified nurses and staff from other professions to support with the delivery of excellent care to some of the most vulnerable patients.

You will be able to travel around the Borough and deliver care in patient's homes.

  • To work as part of a multidisciplinary therapy team delivering a communitybased rehabilitation service to clients with a variety of conditions under the supervision of qualified therapists including Speech and Language Therapists Occupational Therapists and Physiotherapists.
  • To assess and manage own caseload of allocated clients referred with specific, predictable presentations under the supervision of a qualified therapist.
  • To provide nonclinical support to the qualified therapists within the team.
  • To achieve and maintain competencies within the Intermediate Care competency framework for this grade according to Trust and local guidelines.
