Job Title: Rehabilitation Support Worker

We are seeking a skilled and compassionate Rehabilitation Support Worker to join our team at Guy's and St Thomas' NHS Foundation Trust. As a Rehabilitation Support Worker, you will play a vital role in providing high-quality care and support to adults with disabilities or significant health needs in their homes.

Key Responsibilities:
  • Support senior clinicians in the assessment, planning, implementation, and review of individual rehabilitation and care programs for clients.
  • Implement rehabilitation and care interventions to clients in their homes, including teaching prescribed exercise programs, practicing transfers and mobility, and assisting with personal care and domestic tasks.
  • Work autonomously and be accountable for your own work, identifying symptoms of changing conditions among clients and taking timely action.
  • Communicate effectively with clients, their families, and healthcare professionals, particularly those with barriers to communication.
  • Assess and manage clinical, physical, and environmental risks to ensure the safety and well-being of staff and clients.
Requirements:
  • Evidence of literacy and numeracy to GCSE or equivalent.
  • NVQ II or equivalent skills in relevant subjects, such as biology or social care.
  • Previous experience of working with adults with disabilities or significant health needs.
  • Experience in administrative work, such as filing, scanning, and message-taking.
  • Ability to work autonomously and be accountable for your own work.
  • Ability to communicate effectively with clients and their families.
