The Liaison Officer is the pivotal post for the Integrated Community Teams. The Liaison Officer will be interface between primary care, secondary care, community care, social care and voluntary organisations.


The Liaison Officer will have overall responsibility for the coordination of the regular Multi Disciplinary Team meetings and the tracking of patients who are on the caseload of the Integrated Community Team.

This role includes completing all administrative, information and organisational duties to ensure maximal outcomes from meetings and coordinated care for patients.


The key role of the Liaison Officer will be to schedule all meetings acrossall of the general practices in their locality, to manage the meeting agenda, ensuring all new referrals are identified and information circulated to team members.

They will update the service user records contemporaneously on the identified IT systems.

To support the day-to-day operations within the West Neighbourhoods, we are looking to recruit a Case Liaison Officer.

The main responsibility is to coordinate and manage the administrative functions for the integrated care meetings.


Liaise with service users receiving care coordination and interface with all professionals involved in service users care including primary, secondary, the ambulance Service, community services, carers, third sector organisations and other relevant groups.


Take responsibility for tracking patients who are on the caseload for the team, including when patients are admitted to hospital and be in a position to know what is happening with a patient's care as they progress through the pathway.


To develop and sustain effective communication with GP practices and other partners in order to feedback the outcomes of individual referrals including interventions carried out and details of any ongoing support that may be required.


To deal in a professional, helpful and sensitive manner with service users, staff and other agencies by telephone or face to face, taking messages, advising service users about visits, referring other issues as appropriate and answering routine enquiries.

Devising and maintaining efficient filing systems, clerical and office systems, and provide administrative support to the team, including photocopying, faxing, dealing with other forms as required

To support completion of service users referrals, record electronically and feedback to team members

To be proficient in using Microsoft office packages and other identified IT systems and supports other team members in their use


To extract caseload information from various clinical systems, and undertake analysis of information for audit, performance management and capacity purposes, including data entry, retrievals, manipulation and reporting formatting.


To input data onto the service user information management systems to update patient records, including outcomes of reviews, planned interventions/assessments, patient details, and periodically use reports to ensure service user details are correct.

Communicate with other members of the integrated community team if service user at possible need of further intervention to prevent unplanned hospital admission, providing and explaining relevant information regarding services and integrated care to service users and carers
