The Health and Well-being Champion will be part of WYFI Plus covering the Wakefield District.

Working as part of multi-disciplinary team you will deliver high quality support for clients with multiple and complex needs, in line with Spectrum protocols.

The post holder will work with the team to develop positive relationships and care pathways with key stake holders across the district.

Under the supervision of the service manager, the post holder will compliment and form a key part of the Multi-Disciplinary WYFI Plus approach to healthcare providing relevant health and well-being advice and information to client whilst assisting them to navigate access to appropriate services using a trauma informed approach.

The post holder will carry a small caseload while supporting the wider WYFI Plus navigator team, helping clients to access resources and to provide information on health education/promotion.

The Health and Wellbeing Champion will deliver agreed care to clients who are experiencing health inequalities as agreed by the team.

They will provide harm reduction and long term condition advice as part of the district wide health inequalities strategy.

Assisting in the delivery of targeted interventions that aim to improve awareness and management of long term conditions amongst this specific population as identified by the Core20plus5 approach.


The post holder will implement and provide high standards of care to clients under the supervision of the service manager.

They will carry a small caseload while supporting the wider WYFI Plus navigator team, helping clients to access resources and to provide information on health education / promotion.

They will improve clients healthcare needs by assessing, planning, implementing and evaluating individual client care plans.

Providing and offering healthcare advice and information on a range of health needs including BBV, immunisations, basic wound care, nutritional advice, basic sexual health advice, mental health advice, COPD and substance misuse harm reduction.

The health and well-being champion will have an understanding of physical, mental health and substance misuse issues and support clients to understand their health and well-being treatment options.
