This Role focuses on early identification at ward level of patients who may require support with a complex discharge need to include referring to and liaising with the wider multidisciplinary team


The role also focuses on identifying any delays in completion of diagnostic tests to medical teams/service line at the earliest opportunity to prevent any potential delay in discharge planning.


Alongside the ward administrator there is also a requirement to provide a clerical service to the ward and ensure all systems are kept up to date to allow smooth running of the ward.

This post is designed to support the Multi

  • Disciplinary Team by coordinating timely patient assessment in readiness for a safe discharge.


The purpose is that the post will provide one point of contact for all disciplines supporting the patient through the inpatient journey; ensuring that consistent real time information is given to those involved.

Assist in co-ordinating all aspects of the patients discharge, the planning of which will begin on admission.

We are looking for an enthusiastic and energetic individual to join our department in a post that is interesting and varied, providing efficient and safe discharges for our patients.


Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS at Risk staff throughout Devon who is able to display recent relevant experience as dictated by the Person Specification.

  • To discuss estimated dates of discharge with patient/ carers and communicate any concerns to the ward nurse responsible for coordinating the care pathway.
  • Use the patients discharge checklist as a tool for coordinating the patient journey.
  • Ensure that timely referrals are made to the disciplines identified by the ward nurse following identification of issues surrounding discharge.
  • Liaise with patient regarding mode of transport home.
  • Contact relatives/ care provider, on request of patient/ registered nurse coordinator to inform of decision to discharge from hospital and liaise regarding plans.
  • Book transport as directed by registered nurse coordinator based on patient care needs.
  • Keep documentation up to date, ensuring records of all facilitated discharges are recorded.
  • Ensure results or requested diagnostic tests are available for medical teams as soon as possible to enhance clinical decision making.
  • Liaise with multi
- disciplinary team re outcomes of assessments and ensure recommendations are communicated to other agencies in a timely manner. Ensure patients/ carers are aware of outcomes and further recommendations.

  • Finalise arrangements at least 24 hours before discharge, ensuring that transport/ TTA's are available ensuring mínimal delay on day of discharge and any appropriate referrals to primary care i.e district nurses have been made.
  • Liaise with ward pharmacist to ensure prescription of TTA's for those patients whose discharge is the same days is coordinated effectively and timely. Maintain the ward TTA cupboard.
  • Participate in the audit of discharge procedures.
  • Attend daily board meeting with Registered Nurse coordinator to discuss discharge plans with the multi-disciplinary team.
  • Answering telephone enquiries in conjunction with, and under the direction of, nursing staff.
  • Entering admissions and discharges on iPM, including ward transfers, clinical transfers and speciality transfers and updating patient information where necessary.
  • Ensure availability of case notes (including investigation reports) for all admissions.
  • Obtaining notes -from other departments using computerised tracing system.
  • Maintain hospital notes in accordance with the University Hospitals Plymouth NHS Trust
  • Ensure notes are in order when patients are transferred
  • Ensuring that the bed state is kept up to date on iPM and Salus.
  • Liaising with other departments and secretaries regarding appointments, investigations to be arranged and booking transport as required.
  • As required, telephone patients with urgent/last minute appointments.
  • Notify a patients GP regarding patient death.
  • Filing reports when signed by doctor and ensuring notes are kept in accordance to the Plymouth Hospitals NHS Trust Policies.
  • General liaison with other departments under direction of the nurse in charge.
  • Sorting and opening incoming post accordingly.
  • To cover opposing ward areas when requested by the Administration manager, during staff absence or increased workload.
  • To carry out any other duties as determined by the line manager/ward manager.
  • In the absence of the ward administrator be familiar with ordering supplies as requested by the nursing staff through electronic procurement: follow-up outstanding requests for repairs.
