Community Sister/Charge Nurse

About the Role

We are seeking an experienced and skilled Nurse Practitioner to join our Discharge to Assess team at Livewell Southwest. As a key member of our team, you will be responsible for providing high-quality nursing care to patients referred from the acute general hospital setting.

Key Responsibilities

  • Provide holistic evidence-based care to patients in accordance with national and organisational policies and procedures.
  • Work autonomously and unsupervised in the community setting, recognising the risks associated with lone working, aggressive behaviour, and work proactively to reduce those risks.
  • Assess, plan, implement, and evaluate patient care on a day-to-day basis, as part of a multi-professional team.
  • Liaise with planned resources, both internal and external to the organisation, and work in partnership to offer optimum opportunity for people to meet their desired outcomes.
  • Supervise and manage the performance of any staff line managed by this post and provide leadership for support staff through supervision and appraisal.
  • Maintain electronic record keeping and have opportunities for mobile working.

Requirements

  • 2 years + recent experience as a Band 5 Nurse either working in acute or community setting.
  • Ability to demonstrate transferable skills.
  • Experience or understanding of community working.
  • Relevant clinical experience and competence, including medicines management, complex care co-ordination, and end-of-life care.
