Carers Services Project Manager

The post-holder will support Torbay Carers Services Team with defined projects/areas to improve the experience of Carers in Torbay.
Key elements of this role will include supporting the Carers Lead and service managers linking in with partner organisations (existing care providers, community voluntary organisations, and private businesses not currently involved in the care sector) and Carers to meet defined project objectives. The role will also undertake a variety of functions such as Carers Health and Wellbeing checks with Carers who are veterans, or who are caring for veterans, support the delivery of the Carers for Carers volunteer phone line, and support the delivery of breaks for Carers. The role offers a unique opportunity to support the development of services while Torbay Carers Service adapts to ever-changing demands on Carers in Torbay.
The role will include delivering short-term projects on time to achieve strategic demands, while maintaining oversight of the wider Carers Strategy and facilitating its wide-ranging strategic outcomes.
- Supporting Torbay Carers Service to deliver strategic time-defined outcomes in support of Carers.
- Co-producing developments and engaging with a variety of Carers whenever possible. Bringing in knowledge and expertise from the wider Carers community (Carers, Carer Evaluators, partner organisations etc.).
- Working to identify opportunities for Carers to have hotel and other accommodation breaks at reduced cost or free.
- Undertaking Carer support work and Carers Assessments with Armed Forces Veterans and other Carers as needed, who are caring or are being cared for. One-to-one work with Carers to support/signpost.
- Where Care Act eligible needs for additional support are identified, to arrange basic support (eg. Emotional Support Scheme), or to liaise with other teams to do so as appropriate (eg. Zone Teams, Devon Carers), or to refer onto specialist Carer Support Workers (eg. Mental Health, Substance Misuse).
- Managing volunteer coordination role for Carer to Carer Volunteer phone line.
- To support key events for Carers across the year, such as Carers Week, Carers Rights Day and Young Carers Action Day.
- At all times, to ensure that care and support is delivered in accordance with the Care Act 2014, Children and Families Act 2014, NICE guidance .
Torbay Carers Services is a small, dynamic team working hard to identify and support unpaid carers across Torbay. We work closely with many carers, and other teams and partners across Torbay. Job description Job responsibilities Communication and working relationships
- You will be required to communicate with a wide variety of stakeholders in an appropriately clear, timely, and consistent manner, and through a variety of channels (which may include giving presentations or writing reports).
- To use a flexible communication approach which meets the needs of stakeholders and promotes openness and transparency.
- To be confident in working with stakeholders at all levels, but particularly unpaid Carers.
- To work closely with Carers Service team members to deliver timely actions, working with each member of the team as projects workload dictates.
- To raise awareness of Carers, their contribution and needs.
- To act as a point of reference to stakeholders including a range of Torbay and South Devon NHS Foundation Trust employees, Torbay Councillors and Council employees, commissioners and third sector organisations.
- Maintain Trust, staff and patient/client confidentiality at all times (unless safeguarding concerns take precedence).
- Effective written communication.
- Effective communication with staff at all levels both within the Trust and outside organisations.
- Able to communicate effectively in an often noisy and busy environment.
- Ability to search risk management system /other systems for relevant data.
- Analyse data and write reports and papers within scope of role as required.
- Understand the scope of the role and use judgment as to how/when to escalate concerns effectively.
- To manage own diary, ensuring that capacity is used most effectively.
- To be able to work as part of a team, taking direction from the Carers Management Team where necessary.
- To be able to work autonomously and on own initiative as required, prioritising urgent tasks effectively and juggling changing demands.
- To support with relevant evaluation and scoping activity.
- To provide project co-ordination and support, keeping accurate records as appropriate to the role.
- To arrange and attend meetings and events as appropriate to the role, which may include events outside of the local authority area on occasion.
- To arrange and undertake assessments (via phone, Teams, office based or at the individuals home) when required to complete Carers Assessments (Health & Wellbeing Checks).
- To be able to work effectively in an environment which is evolving and uncertain.
- To ensure that the Carers Service Managers are kept up to date on progress, risks and issues which may arise.
- To work with unpaid carers in an effective and supportive way.
- To be accountable to the Carers Lead, Delivery Manager and Development. Manager for the delivery of agreed outcomes within own work area.
- To make decisions as appropriate to role and work area.
- To escalate any risks and issues which may impact the delivery of projects outcomes appropriately and in a timely way.
- To achieve project goals while exploring innovative solutions and working with Carers Service managers to maximise the impact of strategic projects work strands.
- Required to travel to locations across the Trust if required by the public/staff.
- To contribute to the development of strategy, policy, and process as required.
- To support continuous quality improvement.
- To support the delivery of agreed service outcomes.
- Knowledge of national guidelines and policies in relation to Carers.
- Contribute the development of the service through objectives set in annual appraisal and supervision.
- Knowledge of current NHS and social care policies, processes and structures.
- Awareness to national NHS/Social Care initiatives which will impact on patient/client care and treatment.
- Pro-active and consistent in relation to service development ideas.
- To ensure that Trust resources are utilised responsibly.
- To keep financial records as appropriate (e.g., expenses or any costs associated with arranging events, meetings etc.).
- To ensure any expenditures are authorised by the relevant budget holder.
- Responsibility for promoting and role modelling a person-centred, strengths-based, and compassionate approach.
- To advocate for the needs and rights of informal Carers.
- To support from an operational/project perspective the development of local Carers Service provision in Torbay.
- Continuing professional development.
- Required to use a VDU for a significant part of each day.
- Must be able to use word processing packages including Microsoft Word, Excel, Outlook, Teams and a variety of other Trust applications including IHCS and the Trusts Risk Management system.
- Use telephone and email technology effectively.
- To maintain accurate and up to date records.
- To prepare or contribute to presentations or reports as required.
- To support the evaluation of service developments as required.
- Qualifications and training Educated to high level of general education, A Level or equivalent level of experience of working at a similar level in specialist area. oEvidence of continuing professional development.
